Hip pain is a common problem in runners, gym-goers, and active people. It can develop gradually or come on suddenly after an increase in training, change in activity, or overload of the hip muscles and tendons.

What causes hip pain?

Some common contributing factors to sport related hip pain include:

  • Weak or underactive gluteal muscles
  • Reduced single leg stability
  • Sudden changes in training volume or speed
  • Fatigue leading to altered running biomechanics
  • Hormonal influence (eg menopause)

Book an appointment with Podium Physiotherapy to start your recovery and return to training with confidence.

Hip pain in active adults is usually related to overload rather than a single injury.

Common conditions include:

Gluteal Tendinopathy

One of the most common causes of outer hip pain in runners. It involves irritation of the gluteal tendons, which help stabilise the pelvis during running. Often people report pain when lying on one side and when crossing their legs.

Hip Flexor Overload or Strain

Pain at the front of the hip, often linked to sprinting, hill running, or sudden increases in training intensity. This can feel like a tight or pulling sensation at the front of the hip.

Hip Osteoarthritis

Hip arthritis is a degenerative joint condition where the cartilage in the hip gradually wears down, leading to pain, stiffness, and reduced movement. This commonly develops with age or after previous injury, and symptoms often include groin or thigh pain that worsens with activity and improves with rest.

Femoroacetabular Impingement (FAI)

A structural or movement-related issue causing pinching in the front of the hip during running or deep flexion. Symptoms are often reproduced when squatting or sitting in low chairs.

Femoral Neck Stress Fracture

Characterised by a dull ache that is usually felt in the front of the thigh. The pain develops over a period of a few weeks and is frequently made worse by exercise. It may also be felt during walking, rest and even at night.

Adductor Tendinopathy

This is an overuse injury affecting the tendons of the inner thigh muscles, most commonly where they attach near the pubic bone. Adductor tendinopathy typically causes groin pain that is worse with activities such as running, cutting, or kicking, and may feel stiff or tender during movement.
